沖縄へ行くときの楽しみの一つは、普段は見かけない様々な果物に出会うことができることだ。ドラゴンフルーツやシークァーサー、そしてスターフルーツなどなど。
しかしこのスターフルーツは、腎機能障害のある人には危険な食物だ。原因物質は未だ特定されていないが、腎障害患者がスターフルーツを食べると脳症を起こすことがある。もしも脳症が起きれば、20〜30％は死に至ると言われている。腎障害の疑いのある人は、スターフルーツには十分注意すべきだ。

PURPOSE:
Star fruit has been reported to contain neurotoxins that often cause severe neurological complications in patients with uremia or severe chronic renal insufficiency. However, the occurrence of neurotoxicity in patients with mild or moderate renal insufficiency has rarely been mentioned.
CASE REPORT:
A 67-year-old woman who had diabetes mellitus and moderate renal insufficiency presented with acute onset of hiccups and nausea two hours after ingestion of one fresh star fruit. She further presented with progressively incoherent speech, echolalia and bizarre behavior. On the next day, her consciousness level declined to deep coma and she experienced two seizures. Brain magnetic resonance imaging examinations revealed a focal cerebral lesion over the left occipital area. The clinical symptoms recovered and the brain lesion reversed after emergency hemodialysis.
CONCLUSION:
The clinicians should be aware that star fruit intoxication must be considered when patients with a chronic renal disease, even mild or moderate chronic renal insufficiency, present with unexplained neurological or psychiatric symptoms. Emergency hemodialysis or other replacement therapies may be required for the management of acute star fruit intoxication.

可逆性後頭葉白質脳症を呈した、中等度腎不全をともなう患者のスターフルーツ中毒
目的:
スターフルーツは神経毒を含み、しばしば重度の神経学的合併症の原因となると報告されてきた。しかしながら、軽度または中等度の腎不全患者における神経毒性の出現は稀だと言われてきた。
症例報告:
糖尿病と中等度の腎障害を持つ67歳女性が、新鮮なスターフルーツ1個摂取2時間後に、吃逆と嘔気を急に発症した。さらに進行性の会話錯乱、反響言語、奇異行動が認められた。次の日、意識レベルは低下し深昏睡となり、二回の痙攣発作を起こした。脳MRI検査にて、左後頭領域を覆う巣状の大脳病変を認めた。緊急透析後、臨床症状は回復し、脳の病変は戻った。
結論:
臨床医は、たとえ軽度から中等度の慢性腎障害であっても、慢性腎疾患の患者が原因不明の神経学的、精神科的症状を示す場合は、スターフルーツ中毒を考えなければならないことを認識すべきである。急性スターフルーツ中毒の管理には、緊急透析あるいは他の代わりとなる治療がおそらく必要である。
